You'll find your Tattoo Registry on the right side of your stable home page under Upgraded Features. It's the last on the list.

Click on that and it will take you to your registry where you will find your first free tattoo slot and a place to purchase additional slots for 10k each. There's a place to give each slot a specific name, and beside it a link to the tattoo editor (Edit tattoo image). You can design your tattoo there, using a wide range of colors. The tattoo size is 25 pixels x 25 pixels. I usually create a tattoo pixel by pixel and then edit it as I please.
